Job Summary

92NY’s 60+ Programs includes the Himan Brown Program, an all-inclusive membership program for our 60+ community of lifelong learners as well the Program for Cognitive Strength and Ability (PCSA), a cutting-edge, strength-based program for community members with mild to moderate cognitive impairment.

 

The Director of PCSA Community Care and Outreach (CCO) will actively co-direct the 92NY Program for Cognitive Strength and Ability (PCSA) with the Director of PCSA Program Operations.  This role will lead all support and social work services for participants and families as well as coordinate all community focused programming and outreach.

Job Responsibilities Include

  • Collaborate with Executive Director and Director of Program Opersations on program growth and development, strategic direction, and formal program evaluation.
  • Formally assess potential members for PCSA groups.
  • Monitor the well-being and engagement of all PCSA participants.  
  • Primary social worker for PSCA family members.
  • Maintain close, ongoing, and supportive contact with PCSA family members.
  • Support participants and their family members through short term counseling, advocacy, information, and referrals.
  • Facilitate ongoing support groups for participant family members.
  • Serve as initial point of contact for program inquiries and intakes.
  • Develop and host monthly community Zoom workshops (92NY PCSA Family and Friends Support Series) featuring experts in the field.
  • Support development team in efforts to secure and write grants/reports. 

Community Outreach:

  • Develop and maintain relationships with hospital based staff at related departments in NYC,  individual health care providers, community-based agencies, research institutions, geriatric focused organizations and medical practices for member referral and collaborative partnerships.
  • Liaise with social workers in the geriatric field to share best practices and grow referral network within the industry.

Staff Management:

  • Co-manage with Director of Program Operations of all per diem social workers and auxiliary staff.
  • Co-facilitate weekly staff meetings.
  • Develop and manage quarterly staff training.
  • Provide supervision for social work interns as needed.

Experience, Education, & Skills Desired

  • LMSW with a current, active NY State License; LCSW preferred.
  • 5+ years of experience providing clinical Social Work services to adults.
  • Experience working with adults with cognitive loss preferred.
  • Experience facilitating therapeutic, support and psychoeducational groups.
  • Demonstrated leadership experience managing a team of professionals.
  • Demonstrated project management experience with the proven ability to manage, balance and meet multiple deadlines.
  • Experience working in a fast paced, community-focused environment, with the ability to manage the needs of a diverse group of people with ease and diplomacy.
  • Ability to thrive as a team player, often with competing demands.
  • Strong organizational, verbal, and written skills.
  • Familiarity with a range of community resources for older adults and the ability to make effective referrals.
  • An understanding of Jewish traditions and culture a plus.

Work Environment & Requirements

40 hours per week in-person. Hybrid option negotiable. Monday to Friday, 9:00 am to 6:00 pm.

Compensation Range

$80,000 - $95,000

 

The actual compensation offered will be based on a number of factors including, but not limited to the qualifications of the applicant, years of relevant experience, level of education attained, certifications or other professional licenses held, and if applicable, the location in which the applicant lives and/or from which they will be performing the job.
